Insiden berkenaan pada mulanya berlaku selepas pendahulu kelas Moto2, Enea Bastianini terjatuh dengan jenteranya berada di tengah laluan sebelum Hafizh yang tidak menyedari kemalangan itu gagal mengelak dan melanggar motosikal pelumba Italtrans Racing Team berkenaan. Motosikal ditunggang Hafizh dilihat berkecai dan pelumba Openbank Aspar Team itu melambung tinggi di udara. Kemalangan itu turut membabitkan beberapa pelumba lain iaitu Andi Farid Izdihar (IDEMITSU Honda Team Asia) dan Edgar Pons (Federal Oil Gresini Moto2) sebelum saingan terpaksa dikibarkan bendera merah susulan insiden berkenaan. Begitupun, Hafizh dilaporkan berada dalam keadaan sedar dan hanya mengalami lebam di beberapa bahagian badan. Bos pasukan Openbank Aspar Team, Gino Borsoi berkata: "Hafizh berada dalam keadaan baik, dengan dia hanya mendapat lebam terutama di bahagian pinggul tetapi berada dalam keadaan sedar." Sementara itu, Prinsipal Pasukan PETRONAS Sepang Racing Team, Datuk Razlan Razali turut mengesahkan perkara sama. "Hafizh berada dalam keadaan sedar dan hanya lebam. Selainnya OK.
